After much mooching around and reading, it turns out I missed the obvious:

The indexer in EPrints wasn't running so the DOI minting events were all stacked up. Started the indexer and now I have my test server DOIs.

There was nothing wrong with https://mds.test.datacite.org/" and certainly nothing wrong with the plugin.

Just putting this out there if anybody else is stuck on this in the future (or present!).

The last Test deposit with a DOI minted successfully was on 14/11/2018. The next Test deposit in the live archive on 19/02/2019 has no DOI. I did some deposits earlier and didn't appear to get any errors in the logs.

If anybody can shed some light on this I'd be very grateful (I have emailed DataCite support to see if they know of anything).
