I am not familiar with the ProQuest deposit scenario, but the servicedocument xml should be all that any SWORD client needs, including ProQuest.

Can you give me a link to info. on the ProQuest SWORD client/form.

Best to give the servicedocument URL.  The servicedocument is the starting block for all SWORD interactions, and it could vary for different repositories.

For EPrints it is:
http://your.repository/sword-app/servicedocument
